Resumen Introducción: la disfagia se clasifica dentro de los “síntomas y signos que involucran el sistema digestivo y el abdomen” en la Clasificación Internacional de Enfermedades (CIE-10, código R13). Dicho síntoma se produce en la región de la faringe, el esternón o el xifoides como resultado de la obstrucción del paso de alimentos de la boca al estómago o al cardias, y se ha convertido en una de las causas más importantes que afecta a la ingesta adecuada de energía y nutrientes. Un abordaje dietético y nutricional óptimo desempeña un papel crucial en el manejo de la disfagia. El objetivo principal de dicho abordaje es garantizar una alimentación segura y suficiente para prevenir la desnutrición y las complicaciones asociadas a la disfagia. Objetivos: establecer un protocolo de actuación para realizar, de forma eficaz, el abordaje dietético y nutricional en pacientes con disfagia en base a la evidencia científica existente. Métodos: se llevó a cabo una revisión de la literatura científica sobre el abordaje dietético-nutricional de la disfagia. Resultados: el abordaje dietético y nutricional en pacientes con disfagia requiere un enfoque multidisciplinar y personalizado, y es fundamental para mejorar la calidad de vida de los pacientes con disfagia. A través de la implementación del protocolo de actuación basado en la evidencia, se puede garantizar: prevenir la aspiración y asegurar una deglución segura, y modificación de texturas de los diferentes alimentos (promoviendo de esta forma la ingesta adecuada de nutrientes y líquidos para evitar la desnutrición y la deshidratación). Conclusión: un manejo adecuado y protocolizado, desde el punto de vista dietético y nutricional, puede llegar a tener un impacto significativo en la calidad de vida de los pacientes, mejorando su bienestar y previniendo complicaciones asociadas a esta condición.

Abstract Introduction: dysphagia is classified under “symptoms and signs involving the digestive system and abdomen” in the International Classification of Diseases (ICD-10, code R13). It is defined as obstruction in the region of the pharynx, sternum or xiphoid because of obstruction of the passage of food from the mouth to the stomach or cardia, and has become one of the most important causes affecting the adequate intake of energy and nutrients. An optimal dietary and nutritional approach plays a crucial role in the management of dysphagia. The main goal of such an approach is to ensure safe and sufficient nutrition to prevent malnutrition and complications associated with dysphagia. Objectives: to establish an action protocol for an effective dietary and nutritional approach in patients with dysphagia based on the existing scientific evidence. Methods: a review of the scientific literature on the dietary-nutritional approach to dysphagia was carried out. Results: the dietary and nutritional approach in patients with dysphagia requires a multidisciplinary and personalized approach and is essential to improve the quality of life of patients with dysphagia. Through the implementation of an evidence-based action protocol, it is possible to guarantee preventing aspiration and ensuring safe swallowing and modification of textures of different foods (thus promoting adequate intake of nutrients and fluids to avoid malnutrition and dehydration). Conclusion: adequate and protocolized management, from a dietary and nutritional point of view, can have a significant impact on the quality of life of patients, improving their well-being and preventing complications associated with this condition.

Resumen La resistencia a la insulina se explica como un defecto en la unión de la insulina con su receptor y está asociada con numerosas enfermedades, como la obesidad o la diabetes tipo 2, entre otras. La resistencia a la insulina se ha relacionado con la deficiencia de vitaminas y minerales, especialmente de aquellos involucrados en el estrés oxidativo. La dieta mediterránea, una dieta basada en el Healthy Eating Index o la dieta Dietary Approaches to Stop Hypertension (DASH) son patrones dietéticos que se han asociado con un menor riesgo de presentar resistencia a la insulina en edad infantil. Por tanto, una dieta rica en vitaminas y minerales antioxidantes, fibra, calcio y ácidos grasos poliinsaturados y baja en azucares libres, sodio y ácidos grasos saturados puede disminuir el riesgo de presentar resistencia a la insulina en este grupo de edad. Además, otros factores nutricionales, como evitar la comida rápida, cenar en familia, no comer mientras se ve la televisión o el consumo regular de un desayuno suficiente y saludable son hábitos que parecen estar relacionados con menor riesgo de presentar resistencia a la insulina. Por tanto, es importante establecer hábitos alimentarios diarios equilibrados para prevenir y tratar la resistencia a la insulina en escolares y adolescentes.

Abstract Insulin resistance is described as a defect in the binding of insulin to its receptor and is associated with several diseases, including obesity and type 2 diabetes. Insulin resistance has been linked to vitamin and mineral deficiencies, especially those involved in oxidative stress. The Mediterranean diet, a diet based on the Healthy Eating Index or the Dietary Approaches to Stop Hypertension (DASH) diet are dietary patterns that have been associated with a lower risk of developing insulin resistance in children. Therefore, a diet rich in antioxidant vitamins and minerals, fiber, calcium, and polyunsaturated fatty acids and low in free sugars, sodium and saturated fatty acids may decrease the risk of insulin resistance in this age group. In addition, other nutritional factors, such as avoiding fast food, eating dinner with the family, not eating while watching TV or eating a sufficient and healthy breakfast on a regular basis seem to be associated with a lower risk of insulin resistance. Therefore, it is important to establish balanced daily eating habits to prevent and treat insulin resistance in schoolchildren and adolescents.

Resumen La inducción del sueño y su calidad son temas de preocupación creciente porque su deterioro afecta a un número elevado de personas y supone un riesgo en su bienestar y calidad de vida y en la salud a largo plazo. Hay diversos factores implicados en el problema, pero la nutrición es uno de ellos y, en concreto, el consumo de leche se ha relacionado frecuentemente con los hábitos de sueño, a veces como factor promotor y otras como inhibidor. Profundizar en el tema es el objeto de la presente revisión. El triptófano, al llegar al cerebro, es la base para la síntesis de serotonina y melatonina, que mejoran la inducción y la calidad del sueño. Pero hay una competencia entre el triptófano y otros aminoácidos neutros de cadena larga (ANCL) (valina, leucina, isoleucina, tirosina y fenilalanina) para cruzar la barrera hematoencefálica y llegar al cerebro. En este sentido, las proteínas de la leche, con elevado contenido en triptófano y la relación más elevada entre triptófano y ANCL, son muy útiles en la promoción del sueño. Por otra parte, la leche también aporta diversos micronutrientes que ayudan en la transformación del triptófano en serotonina y melatonina, así como componentes antioxidantes, antiinflamatorios y péptidos bioactivos, y estudios recientes indican que modula favorablemente la composición de la microbiota intestinal. Los estudios realizados ponen de relieve que aumentar el consumo de leche, hasta el aporte aconsejado y dentro de una alimentación correcta, favorece el conseguir y mantener un sueño de calidad.

Abstract Sleep induction and its quality are issues of growing concern because its deterioration affects a large number of people and poses a risk to their well-being and quality of life and long-term health. There are several factors involved in the problem, but nutrition is one of them and in particular milk consumption has often been linked to sleep habits, sometimes as a promoter and sometimes as an inhibitor. The purpose of this review is to examine the matter further. On reaching the brain, tryptophan is the basis for the synthesis of serotonin and melatonin, which improve the induction and quality of sleep. But there is competition between tryptophan and other long-chain neutral amino acids (LNAA) (valine, leucine, isoleucine, tyrosine and phenylalanine) to cross the blood-brain barrier and reach the brain. In this sense, milk proteins with a high tryptophan content and the highest ratio between tryptophan and LNAA are very useful in promoting sleep. Moreover, milk also provides various micronutrients that help in the transformation of tryptophan into serotonin and melatonin, as well as antioxidant components, anti-inflammatory and bioactive peptides, and recent studies indicate that it favorably modulates the composition of the intestinal microbiota. Studies show that increasing milk consumption, up to the recommended intake and within a correct diet, favors the achievement and maintenance of quality sleep.

Resumen Está aumentando el número de personas que siguen dietas basadas en vegetales en mayor o menor grado, pero cuanto más restrictiva es una dieta, más riesgos existen de presentar deficiencias nutricionales. La etapa infantil es una etapa de gran crecimiento físico y madurativo, por ello, las deficiencias nutricionales en esta etapa pueden acarrear consecuencias graves y, en ocasiones, permanentes. Las personas que siguen dietas restrictivas de cualquier tipo, como son las dietas basadas en vegetales, deben recibir apoyo de personal especializado para conseguir que sus dietas sean variadas y equilibradas, como es deseable en cualquier caso.

Abstract The number of people following plant-based diets to a greater or lesser degree is increasing, but the more restrictive a diet, the greater the risk of nutritional deficiencies. The childhood stage is a stage of great physical and maturational growth, therefore, nutritional deficiencies at this stage can have serious and sometimes permanent consequences. People who follow restrictive diets of any kind, such as plant-based diets, should receive support from specialized staff to ensure that their diets are varied and balanced, as is desirable in any case.

Resumen Los adultos jóvenes, en su paso desde la adolescencia hacia la vida adulta, deben afrontar cambios físicos, sociales y personales que pueden afectar a su salud. Sus requerimientos energéticos son menores que durante la adolescencia, pero pueden necesitar la misma cantidad o mayor de otros nutrientes esenciales, como folatos, vitamina C, vitamina D, calcio y hierro. Además, su estilo de vida cada vez más sedentario ha llevado a una alta prevalencia de sobrepeso y obesidad en este grupo. Sin embargo, a pesar de ser una etapa sensible a los problemas nutricionales, no se le ha prestado suficiente atención desde este punto de vista. En este contexto, el huevo destaca como un alimento de gran valor nutricional para los adultos jóvenes, dado que es una excelente fuente de proteínas, vitaminas y minerales. Además, el huevo es un alimento de alta densidad nutricional, lo que lo hace especialmente útil en el control de peso y en situaciones de bajas necesidades energéticas pero altas con respecto a otros nutrientes. El consumo de huevo en cantidades moderadas y como parte de una dieta equilibrada ayuda a seguir una dieta saludable en este grupo de población, lo que contribuye a mejorar su salud actual y futura.

Abstract Young adults move from adolescence into adulthood, and they face physical, social and personal changes that can affect their health. Regarding their nutritional needs, their energy requirements are lower than during adolescence, but they may need the same or higher amounts of other essential nutrients, such as folate, vitamin C, vitamin D, calcium, and iron. Furthermore, their increasingly sedentary lifestyle has led to a high prevalence of overweight and obesity in this group. However, despite being a vulnerable stage to nutritional problems, not enough attention has been paid to it from this point of view. In this context, eggs stand out as a food of great nutritional value for young adults, as they are an excellent source of protein, vitamins, and minerals. In addition, eggs are a nutritionally dense food, which makes them particularly useful in weight control and in situations with low energy requirements but high demands for other nutrients. Moderate egg consumption as part of a balanced diet helps to follow a healthy diet in this population group, contributing to improve their current and future health.

Abstract Vitamin D is a key nutrient for health. Recent research has shown that it is not only necessary for the maintenance of bone health, but also for the prevention of cardiovascular diseases, insulin resistance, respiratory diseases, infections, and cancer, among others. Solar synthesis of vitamin D is usually insufficient, so it is necessary to provide enough vitamin D through the diet in order to maintain an adequate nutritional status. Studies carried out in Spain and other countries have shown that serum levels of the vitamin were usually insufficient and that average dietary intakes were well below those marked as recommended, while the main food sources of vitamin D were fatty fish, breakfast cereals, eggs, and dairy. Food sources of vitamin D are scarce, and it is naturally located in the fatty part of foods of animal origin, and in the liver and viscera. Fortifying foods that are not natural food sources of vitamin D but are regularly consumed by a significant percentage of the population, is a strategy that could help to increase vitamin D intake. In this regard, dairy products and breakfast cereals are two of the most widely used food matrices for vitamin D fortification, and their incorporation into the usual diet has proven to be an effective strategy to improve the nutritional situation of the population in vitamin D.

Resumen La vitamina D es clave en la salud y sus funciones se relacionan con el mantenimiento de la salud ósea, cardiovascular, la resistencia a la insulina, enfermedades respiratorias, infecciones y cáncer, entre otras. La síntesis solar de la vitamina D es con frecuencia insuficiente, por lo que para mantener un estatus adecuado de la vitamina es necesario que se aporten cantidades suficientes con la dieta. Los estudios realizados en España y en otros países muestran que los niveles séricos de la vitamina son con frecuencia insuficientes y que las ingestas dietéticas medias se encuentran muy por debajo de las marcadas como recomendadas. Las principales fuentes alimentarias de la vitamina D son los pescados grasos, los cereales de desayuno, los huevos y los lácteos. Las fuentes alimentarias de vitamina D son escasas y se encuentra especialmente en la parte grasa de alimentos de origen animal, en hígados y en vísceras. El enriquecimiento de alimentos que de forma natural no tienen un elevado contenido en vitamina D, pero que son consumidos habitualmente por un porcentaje significativo de la población, es una estrategia que ayuda a aumentar su ingesta. En este sentido, los lácteos y los cereales de desayuno son dos de las matrices alimentarias más utilizadas y su incorporación en la dieta habitual ha demostrado ser una estrategia eficaz para mejorar la situación nutricional de la población respecto a la vitamina D.

Abstract In recent decades, there has been an increasing interest in vegetarian diets. It is therefore interesting to analyse the benefits and risks of these diets. To this end, a review has been done on the benefits and risks of vegetarian diets. Vegetarian diets have been associated with different health benefits. On the contrary, nutritional deficiencies has been observed in those people who follow this type of diet, mainly in those risk groups such as pregnant and lactating women, children, adolescents, and elderly. Vegetarian diets may have some health benefits, however further studies are required.

Resumen En las últimas décadas se ha observado un incremento en el interés por el seguimiento de las dietas vegetarianas en la población, por lo que es interesante analizar sus beneficios y sus riesgos. Con este fin se ha realizado una revisión sobre los beneficios y los riesgos de las dietas vegetarianas. Las dietas vegetarianas se han asociado a diferentes beneficios para la salud. De forma contraria, se observan deficiencias nutricionales en aquellas personas que siguen este tipo de dietas, principalmente en grupos de riesgo como mujeres embarazadas y lactantes, niños, adolescentes y ancianos. Las dietas vegetarianas podrían tener algunos beneficios en relación con la salud, sin embargo, es necesario continuar realizando estudios.

Abstract Due to their composition, whole grain cereals allow to achieve the recommended intakes of numerous nutrients that are usually ingested in insufficient amounts according to various studies. On the other hand, scientific evidence indicates that the consumption of whole grain cereals is associated with positive effects on health against the suffering of some chronic non-communicable diseases such as obesity, cardiovascular disease, diabetes or cancer. These effects may be due to their high content of vitamins, minerals, fiber and bioactive compounds. The consumption of whole grain cereals is low, which could be due to the lack of knowledge of its beneficial effects by the population. The consumption of 3 or more servings a day of whole grain cereals contributes to achieve a better nutritional and health status of individuals from early ages.

Resumen Por su composición los cereales de grano entero permiten alcanzar las ingestas recomendadas de numerosos nutrientes que habitualmente se ingieren en cantidades insuficientes, según diversos estudios. Por otro lado, la evidencia científica señala que el consumo de cereales de grano entero se asocia a efectos positivos para la salud frente al padecimiento de algunas enfermedades crónicas no transmisibles como la obesidad, la enfermedad cardiovascular, la diabetes o el cáncer. Estos efectos pueden deberse al elevado contenido en vitaminas, minerales, fibra y compuestos bioactivos que contienen. El consumo de cereales de grano entero es escaso, lo que podría deberse al desconocimiento de sus efectos beneficiosos por parte de la población. El consumo de 3 o más raciones al día de cereales de grano entero contribuye a conseguir una mejor situación nutricional y sanitaria de los individuos desde edades tempranas.

RESUMEN Objetivo Determinar la prevalencia del síndrome de burnout en el personal de enfermería en hospitales del Departamento del Atlántico (Colombia). Materiales y Métodos Se realizó un estudio cuantitativo, descriptivo transversal, en 117 profesionales y 229 auxiliares de enfermería de cuatro hospitales del Departamento del Atlántico (Colombia), dos públicos y dos privados. Se empleó una encuesta anónima con datos sociodemográficos y la escala de Maslach. Resultados La prevalencia global del síndrome de burnout o desgaste profesional fue de 65%. En profesionales de enfermería, fue de 63,2% y en auxiliares de enfermería, de 65,9%. El 13,3% de los participantes presentó un grado elevado de burnout para el cansancio emocional; el 9,2%, para la despersonalización; y el 62,7% obtuvo puntuaciones bajas para la realización personal. En los profesionales de enfermería, el 12,7% obtuvo una puntuación alta para cansancio emocional; 7,4% para despersonalización, y 64,2% obtuvo puntuaciones bajas para la realización personal. Por su parte, en los auxiliares de enfermería el comportamiento en algunas subescalas fue muy similar; el 14,5% obtuvo una puntuación alta para cansancio emocional; 12,8%, para despersonalización y 59,8% obtuvo puntuaciones bajas para la realización personal. Conclusiones La presencia del síndrome de burnout en la población estudiada es alta. Es similar tanto en enfermeros como en auxiliares de enfermería. La dimensión más afectada fue la realización personal en ambos grupos estudiados, lo cual corresponde a sentimientos altos del "quemado".

ABSTRACT Objective To determine the prevalence of burnout syndrome in the nursing staff in four hospitals located in the State of Atlántico (Colombia). Material and Methods A descriptive cross-sectional study was conducted and included as participants 117 nursing professionals and 229 nursing auxiliaries from four hospitals located in the State of Atlantico (Colombia), two public and two privates. An anonymous self-study survey was used on sociodemographic, labor data and Maslach Scale. Results The overall prevalence of Burnout Syndrome or professional burnout was 65%. In nursing professionals it was 63,2% and in nursing auxiliaries 65,9%. At least 13,3% of the participants had a high degree of Burnout from emotional exhaustion, 9.2% for depersonalization, and 62,7% had low scores for personal fulfillment. In nursing professionals, 12,7% had a high score for emotional exhaustion, 7,4% for depersonalization, and 64,2% obtained low scores for personal fulfillment. On the other hand, in nursing auxiliaries the behavior in some subscales was very similar, 14,5% had a high score for emotional exhaustion, 12,8% for depersonalization and 59,8% had low scores for personal fulfillment. Conclusions The presence of burnout syndrome is high in the studied population, it is similar in both, nurses and nursing auxiliaries. The most affected dimension was the personal fulfillment in both groups, which corresponds to high feelings of the "burned".

ABSTRACT In the Chemical Engineering career, laboratory practices require students to have prior knowledge and certain skills for their development, being the operative techniques those that generally hinder the acquisition of knowledge. Taking into account the above and the fact of the application of the TICs curricular strategy in the subjects, this article aims to show the potentialities of the inverted class as an alternative for the development of the Organic and Biological Chemistry laboratories. The theoretical aspects of the flipped class are presented, in which the student is the protagonist of their learning before, during and after the experiments, promoting meaningful learning compared to traditional methods. Its use is exemplified in the experimental activity, identification of multiple links, in a laboratory of the subject, given to the Course by Encounters, presenting at each stage the activities to be developed by the student, and finally the advantages of working on the subject with this alternative. It is concluded that the subject is linked with the TICs curricular strategy, while on the basis of autonomous and collaborative learning, the levels of the cognitive domain of the well-known Bloom Taxonomy are applied according to the digital age, from its previous preparation with the video viewing with the pre-worksheet, to the most effective way of learning in a real field (to do), teaching others in a creative way by designing and editing your video with your mobile phone camera.

RESUMEN En la carrera de Ingeniería Química las prácticas de laboratorio requieren que los estudiantes tengan conocimientos previos y ciertas habilidades para su desarrollo, siendo las técnicas operatorias las que generalmente dificultan la adquisición de conocimientos. Teniendo en cuenta lo anterior y la aplicación de la estrategia curricular TICs en las asignaturas, este artículo tiene como objetivo mostrar las potencialidades de la clase invertida como alternativa para el desarrollo de los laboratorios de Química Orgánica y Biológica. Se presentan los aspectos teóricos de la clase invertida, en la cual el estudiante es protagonista de su aprendizaje antes, durante y después de los experimentos, fomentando el aprendizaje significativo en comparación con los métodos tradicionales. Se ejemplifica su uso en la actividad experimental, identificación de enlaces múltiples, en un laboratorio de la asignatura, impartido al Curso por Encuentros, presentando en cada etapa las actividades a desarrollar por el estudiante, y por último las ventajas que supone trabajar en la asignatura con esta alternativa. Se concluye que se vincula la asignatura con la estrategia curricular TICs, a la vez que sobre la base del aprendizaje autónomo y colaborativo se aplican los niveles del dominio cognitivo de la conocida Taxonomía de Bloom acorde a la era digital, desde su preparación previa con el visionaje del video con la hoja de trabajo previo, hasta la forma más efectiva del aprendizaje en un ámbito real (hacer), enseñando a otros de manera creativa mediante el diseño y edición de su video con la cámara de su teléfono móvil.

Abstract Childhood obesity is an important public health problem, also in Spain. The ALADINO study, in the context of the COSI strategy of the WHO European office, monitors the weight status of Spanish schoolchildren aged 6 to 9 years, and analyzes the associated factors. Data from the 2019 wave indicate that 23.3 % of schoolchildren are overweight, and 17.3 % are obese. A high percentage of schoolchildren eat breakfast regularly, although breakfast quality can be improved. The frequency of consumption of fruits and vegetables is low, even lower than that of other countries participating in the COSI strategy. In terms of lifestyle, more than 70 % of schoolchildren are active, although this percentage is lower than the European average. Screen exposure time is also shorter than in other countries, but this habit may be improved in a high percentage of schoolchildren. There is a lower percentage of schoolchildren with obesity who have breakfast or are active compared to those of normal weight, while they are more sedentary, spend fewer hours sleeping, and are more likely to have a TV or electronic devices in their room. It is a priority to promote access to healthier diets and the promotion of more active and less sedentary lifestyles, both in the family and at school, from an early age.

Resumen La obesidad infantil es un problema de salud pública importante, también en España. El estudio ALADINO, en el contexto de la estrategia COSI de la Oficina Europea de la OMS, monitoriza la situación ponderal de los escolares españoles de 6 a 9 años y analiza los factores asociados al padecimiento de sobrepeso y obesidad. Los datos de la oleada de 2019 indican que un 23,3 % de los escolares tienen sobrepeso y un 17,3 % tienen obesidad. Un porcentaje elevado de escolares desayuna habitualmente, aunque la calidad de esta comida es muy mejorable. Se constata que la frecuencia de consumo de frutas y verduras es baja e inferior a la de otros países participantes en la estrategia COSI. En cuanto al estilo de vida, más del 70 % de los escolares son activos, aunque este porcentaje es inferior a la media europea. También es menor el tiempo de exposición a pantallas que en otros países, aunque mejorable en un porcentaje elevado de escolares. Hay un menor porcentaje de escolares con obesidad que desayunan o son activos en comparación con los que tiene peso normal, mientras que son más sedentarios, dedican menos horas a dormir, y disponen con mayor probabilidad de TV o dispositivos electrónicos en su habitación. Es prioritario favorecer el acceso a dietas más saludables y la promoción de estilos de vida más activos y menos sedentarios, tanto en el entorno familiar como en el escolar, desde edades tempranas.

Abstract Zinc is an essential trace element involved in catalytic, structural, and regulatory functions, participating also in the metabolism of nutrients. Its role in insulin resistance appears to be of great importance, as it is involved in oxidative stress, the endocrine regulation of insulin, and the regulation of glucose uptake. For this reason, both intake and serum levels of zinc are inversely related to insulin resistance. Currently, zinc intake in children is low, and zinc deficiency exists in a significant proportion. Zinc can be found in meat, fish, eggs, and dairy products. Although it certainly is present in foods of plant origin, their high phytate contents generally reduce its bioavailability. For this reason, it is important to monitor intake throughout childhood, and to consider supplementation in children with insulin resistance or obesity.

Resumen El zinc es un oligoelemento esencial involucrado en funciones catalíticas, estructurales y de regulación, además de participar en el metabolismo de los nutrientes. Su función en la resistencia a la insulina parece tener gran importancia, ya que tiene relación con el estrés oxidativo, la regulación endocrina de la insulina y la regulación de la captación de glucosa. Por esta razón, tanto la ingesta como los niveles séricos de zinc se relacionan inversamente con la resistencia a la insulina. A día de hoy, la ingesta de zinc es insuficiente en muchos niños y existe deficiencia de este mineral en una importante proporción. El zinc lo podemos encontrar en carnes, pescados, huevos y lácteos. Aunque es cierto que también existe en los alimentos de origen vegetal, generalmente, su alto contenido en fitatos reduce la biodisponibilidad del zinc. Por esta razón es importante vigilar la ingesta a lo largo de toda la etapa infantil y considerar la suplementación en los niños con resistencia a la insulina u obesidad.

Abstract Malnutrition in pediatric patients conditions their health/illness. Malnutrition in sick children further aggravates their baseline situation, and conditions their evolution and prognosis. Despite its relevance, it is not evaluated in many cases, nor is there a single definition that facilitates its diagnosis. The objective of this article is to establish the principal aspects to consider when evaluating malnutrition, and to highlight the importance of routine evaluation.

Resumen La desnutrición en el paciente pediátrico condiciona su salud/enfermedad. La desnutrición en el niño enfermo agrava aun más, si cabe, su situación de base y condiciona su evolución y pronóstico. A pesar de su relevancia, no se evalúa en muchos casos ni se tiene una única definición que facilite su diagnóstico. El objetivo de este artículo es establecer los aspectos prioritarios a la hora de evaluar la desnutrición y resaltar la importancia de hacerlo de forma rutinaria.

Objective: The aim of this study was to develop and validate a stepwise tool to aid primary health care professionals in the process of deprescribing potentially inappropriate medication in older persons. Methods: We carried out a systematic review to identify previously published tools. A composite proposal of algorithm was made by following the steps from clinical experience to deprescribe medications. A 2-round electronic Delphi method was conducted to establish consensus. Eighteen experts from different countries (Colombia, Spain and Argentina) accepted to be part of the panel representing geriatricians, internists, endocrinologist, general practitioners, pharmacologists, clinical pharmacists, family physicians and nurses. Panel members were asked to mark a Likert Scale from 1 to 9 points (1= strongly disagree, 9= strongly agree). The content validity ratio, item-level content validity, and Fleiss’ Kappa statistics was measured to establish reliability. The same voting method was used for round 2. Results: A 7-question algorithm was proposed. Each question was part of a domain and conduct into a decision. In round 1, a consensus was not reached but statements were grouped and organized. In round 2, the tool met consensus. The inter-rater reliability was between substantial and almost perfect for questions with Kappa=0.77 (95% CI 0.60-0.93), for domains with Kappa= 0.73 (95%CI 0.60-0.86) and for decisions with Kappa= 0.97 (95%CI 0.90-1.00). Conclusions: This is a novel tool that captures and supports healthcare professionals in clinical decision-making for deprescribing potentially inappropriate medication. This includes patient's and caregiver's preferences about medication. This tool will help to standardize care and provide guidance on the prescribing/deprescribing process of older persons’ medications. Also, it provides a holistic way to reduce polypharmacy and inappropriate medications in clinical practice.

Abstract The microbiota of our body is a rising issue on which more and more research is being studied and investigated. The acquisition and modification of the microbiota throughout life, and especially the alteration of its balance, is related to different diseases. One way to modify this microbiota, which can also be useful in disease situations, to restore the proper balance is through the consumption of marketed probiotics. There are very different types of probiotics and not all of them have demonstrated effectiveness in all indications or situations. Therefore, the objective of this article is to explain those indications of probiotics for which evidence has been described according to different guides and scientific studies.

Resumen La microbiota que existe en todo nuestro organismo es un tema de actualidad sobre el que cada vez se estudia e investiga más. La adquisición y modificación de la microbiota a lo largo de la vida, y especialmente la alteración de su equilibrio, está relacionada con diferentes enfermedades. Una manera de modificar esta microbiota, y que puede ser útil en situaciones de enfermedad, para restablecer el adecuado equilibrio es a través del consumo de probióticos comercializados. Existen muy diferentes tipos de probióticos y no todos han demostrado efectividad en todas las indicaciones ni situaciones, por lo tanto, el objetivo de este artículo es explicar aquellas indicaciones de los probióticos para las que se ha descrito evidencia según diferentes guías y estudios científicos.
